Northwest Community Care Network (NCCN) is a network of primary care providers committed to delivering excellent medical care for the Medicaid population in northwest North Carolina.

NCCN Areas of Focus:

• Local care managers who work closely with provider practices, enrollees, and local community organizations.
• A system-wide education effort to help patients select the best place to receive care based on their present health status.
• Utilization of approved disease management protocols in an effort to optimize patient outcomes and reduce unnecessary hospital admissions and emergency room encounters.
